5 signs your back pain needs a physiotherapist

Most episodes of low back pain settle within a few weeks with gentle movement and time. But some signs suggest it is worth getting a proper assessment rather than waiting it out. Here are five.

1. Pain that travels down your leg

Back pain that stays local is usually straightforward. Pain, tingling, or numbness that travels below the knee suggests the nerve is involved — often sciatica — and benefits from a targeted plan.

2. It keeps coming back

If your back “goes” every few months, each episode is a clue that something underneath — often weakness, stiffness, or a movement habit — has not been addressed. Physiotherapy targets the cause, not just the current flare-up.

3. It’s not improving after 2–3 weeks

A little improvement week on week is normal. Pain that is stuck at the same level after two to three weeks is worth assessing, so you are not losing months to something treatable.

4. It’s affecting your sleep or work

Pain that disturbs your sleep or stops you doing your job is having a real impact on your life and recovery. Getting on top of it early usually means a faster, smoother recovery.

5. You’re avoiding activity out of fear

Many people start avoiding movement because they are worried about “damage.” For most back pain, hurt does not mean harm, and careful movement is part of the cure. A physiotherapist can help you move with confidence again.

Red flags — seek urgent care

Separately, get urgent medical attention if back pain comes with numbness around the groin, loss of bladder or bowel control, progressive leg weakness, fever, or unexplained weight loss. These are uncommon but need prompt assessment.
